Introduction
The Hemoglobin HbA1c Poor Control (>9%) protocol is available in Canvas to make care teams aware of recommended services and interventions. Protocols are located on the top right menu bar in the patient chart. A red badge indicates a protocol due.
Key Objectives:

Patient Population:
- Patients 18-75 years of age with diabetes mellitus.
Measure Description:
- Percentage of patients aged 18-75 years of age with diabetes mellitus who had hemoglobin A1c > 9.0% or no test for A1c during the measurement year.
Exclusions:
- Exclude patients who were in hospice care during the measurement year.
Intervention:
- Order HgbA1c and/or intervene with behavioral recommendation and/or medication intensification and follow up testing. A1c may be completed via POC testing.
- Lab results uploaded/come in via Data Integration and must also be reviewed by the provider to complete the intervention. If entering a historical HgbA1c via Data Integration, a lab report with a recorded results linked to the 4548-4 LOINC code must be used. These include:
- Hemoglobin A1C
- Hemoglobin (Hb) A1C With Eag
