Adds new features to letter note type

Letters can now be previewed before printing and faxing. In addition, Letters also display the actions and fax status in the note header along with warnings for any templated variables included in the letter that are missing from the chart (for example if the template includes {{PATIENT_ADDRESS}} but there is no patient address on file).

Adds collapsable accordion filters in patient registration

Patient demographics, preferences, consents, care team, addresses, phone numbers, email, and contact sections can be individually collapsed. All sections are initially expanded by default.

Fixes a bug that could cause double payments

Previously, if a user rapidly clicked more than once when collecting a payment, the payment could be captured twice in Canvas. This fix prevents any additional requests to be submitted when the user clicks more than once.
